G.C.A.P. Enrollment Forms

The following forms must be completed prior to enrollment in G.C.A.P.

All parents are required to read through our Parent Handbook to familiarize themselves with G.C.A.P. policies and procedures. You can read and download the handbook here. You'll also receive a hard copy with your welcome folder. Once you have read through the handbook, please fill out this form:

- Handbook Acknowledgment Form

If your child has allergies or any health conditions that require special attention or medication, please fill out these forms as well:
